Plot[]
The episode opens with Dr. Will Halstead and Dr. Natalie Manning attending pre-Cana, i.e. Catholic pre-wedding counselling from Father Brady. Will is interrupted by constant messages from his brother, Chicago P.D. officer Jay Halstead, asking him to meet regarding the investigation for which Will had previously agreed to act as an informant. Will lies to Natalie, who is unaware of the investigation, and tells her has to run some errands, before going to meet Jay. Jay tells Will that he needs to go meet Ray Burke. Will had rented a wedding venue from Ray Burke, not knowing that he is being investigated by the Chicago P.D. and federal authorities for crimes including money laundering. Jay wants Will to plant a hidden microphone in Ray's house, allowing Chicago P.D. and the Feds to spy on him.
At the hospital, Dr. Connor Rhodes treats an incoming patient, a 49 year old man called Keith Yates, who was in a car accident. Keith is unconscious and has multiple fractures and injuries. Paramedics tell Connor that Keith hit a stanchion head on, and that he didn't appear to be wearing a seatbelt, nor had airbags deployed. Keith wakes up as Connor starts treating him, screaming with pain. He starts struggling to get off the table and attempts to stab himself with a scalpel that he grabs, and tells Connor that he wants to die. Connor tells Sharon that he should have taken precautions as soon as he heard about the airbag, and tells them that Keith has no family. Dr. Ava Bekker, there for a consult, wants to take him for surgery right away, but Connor tells them both that the patient has refused consent for treatment. Ava moves Keith upstairs, so that Dr. Daniel Charles, the head of psychiatry, can work with him. Meanwhile, Connor asks Ava to sign a petition to the board have a new bypass machine for his hybrid O.R. When Ava expresses doubts that the board will approve the funding, Connor tells her he is hoping that they might instead disclose the identity of the anonymous donor who provided the original funding for the O.R., so Connor can ask for more funds. Ava balks at this, telling Connor that she though the donor wanted to remain anonymous. Connor thinks that the donor will be flattered when he goes grovelling for more funds, and Ava signs the petition, looking troubled.
Upstairs, Dr. Charles speaks with Keith, and notices that he has trouble articulating words, saying 'freeze' instead of 'please'. Keith experiences muscle spasms, and Dr. Charles identifies Keith's illness as Huntington's. Keith admits the diagnosis, telling Daniel that there is no cure and that he wants to die before his Huntington's progresses further. Sharon Goodwin accordingly advises Ava that Keith has Huntington's disease, and Ava expresses sympathy for him. They agree that given the condition and his lack of family, they've decided to make him comfortable and not intervene surgically. Ava asks Sharon if she's heard about Connor's petition, and that he intends to contact the lead donor directly. Sharon tells Ava it is hospital policy not to disclose the identity of donors who want to stay anonymous. When Ava asks if she is sure, Sharon asks if Ava is concerned about how Connor will react if he finds out that Ava asked Connor's father for the money, revealing Cornelius Rhodes to be the anonymous donor. Ava says she would prefer it if Connor did not find out. Just then, a man identifying himself as Keith Yates' son, Max Yates, along with his fiance, Elodie. When Ava is surprised, Max tells her that they have been in regular contact, and that they spoke just the day before. Ava tells them that Keith has refused visitors, and when Max pleads, tells her she will do what she can.
As Keith is coding, Ava goes to attend to him, and tells him his son is here. Keith refuses to see his son, and Ava realises that Keith was hoping to die before Max found out about his Huntington's. Dr. Daniel Charles sees Ava tending to Keith and reminds her that Keith has refused surgery and has a do-not-resuscitate order in place. Ava ignores him, telling him that 'do no resuscitate' does not mean 'do not treat'. Daniel tells Ava to let Keith go, but she ignores him. Ava stabilizes Keith and goes to tell Dr. Charles that Keith's son has arrived and wants to meet his father. She tells Dr. Charles that Max could have inherited the disease and they need to test him, but Daniel tells her that they cannot tell Max about Keith's condition due to patient privacy.
At the hospital, Emily Choi has come in for a scan as she is pregnant with a child, along with her boyfriend, Bernard Kim. Emily and her brother, Dr. Ethan Choi, talk, and Emily thanks Ethan for giving Bernard a chance, despite his reservations about their age difference and the fact that they are both addicts in recovery. Later, Maggie Lockwood assigns Ethan to a man who is showing signs of acute alcohol toxicity, and Ethan asks her if a resident couldn't have handled it. Maggie tells Ethan to take the case, and the patient turns out to be Bernie, who has fallen off the wagon, and relapsed into alcohol addiction. Bernie vomits on Ethan. Ethan and April treat Bernie, who apologises, telling Ethan he feels humiliated, and feels like a loser. When he urges Ethan to call him a loser, Ethan tells him he has a lot on his plate. Bernie pleads with Ethan not to tell Emily he lapsed into addiction, and Ethan expresses shock that Bernie intends to hide it from her. A woman arrives for Bernie, providing them with details of his medical history, including the fact that he has pancreatitis and has been warned never to drink again. She introduces herself as Bernie's wife, Ashley Kim, and with her, their teenage daughter, Jamie Kim. A visibly angry Ethan leaves the room.
Later, Keith talks to Ashley Kim, who lets him know that they have actually bought a new house, which might have caused some stress. April tells Ethan that he has a conflict of interest and needs to hand over Bernie Kim to someone else. Ethan instead confronts Bernie about his lies. Bernie insists that he plans on leaving his wife for Emily, and just hasn't told her. Ethan is furious, and tells April outside that if Bernie won't tell Emily the truth about his wife, daughter, and the new house they bought together, then he will. April reminds him that he cannot, because of patient confidentiality. She tells Ethan that she is protecting him, not Bernie. Later, Emily returns to the hospital, and tells Ethan that she was called back by Nurse Leah to fill in some claim forms, but is confused about why she couldn't have done it earlier. Ethan tells her it must be an issue with Billing, and that he will sort it out. While she's there, Emily sees Bernie and talks to him. April asks Ethan what he did, but Ethan doesn't respond. He goes into the room and asks Bernie to tell Emily what is going on. Just then, Bernie's wife Ashley enters. While Ashley and Emily demand answers from Bernie, Bernie collapses.
Later, Ethan sees Ashley berating Emily in the waiting room, calling her a home-wrecker. Emily leaves, running away. Ethan tells Ashley that they had to operate on Bernie, and that he will have a lengthy recovery, but Ashley leaves, telling him she is leaving Bernie, and that Bernie is now Emily's problem. Later, Ethan and April meet Emily, who has returned to Bernie. Ethan tells Emily that he's sorry she found out that way, but now she can plan for life without him. Emily tells Ethan that she isn't leaving Bernie, and that while he might have lied to her, Ethan did too, by contriving an excuse to bring her to the hospital and force the confrontation revealing his secret. Emily tells Ethan that he humiliated her, and jeopardized Bernie's health. When April tries to tell her Ethan was motivated by care for her, Emily shakes her head and leaves.
Returning to the hospital, Natalie tells Maggie Lockwood that she is concerned about Will, who seems to be distracted. Maggie reassures her, telling her it is probably just pre-wedding jitters. Natalie is assigned to a patient, Neela Griggs, a young girl brought in by her mother, Tonya Griggs. While Neela insists she doesn't need help, Tonya tells Natalie that Neela came home late the previous night with bruising on her neck, and went to bed, without talking to her parents, and that Tonya found her crying in the morning, but refusing to share what happened. Neela tells Natalie that she was just playing around, and fell, and Natalie asks Tonya to wait outside. While they help her change, Natalie notices more bruising on her back. Later, Natalie tells Tonya Griggs that while Neela is not concussed, her bruising is inconsistent with a fall, as Neela had claimed. She tells Tonya that the bruises look consistent with being held down forcefully, and that she suspects sexual assault, although they will need to do a pelvic exam to confirm. Tonya tells her that she feared this was the truth as well, and bursts into tears.
When Tonya and Natalie go to speak with Neela, Neela is insistent that nothing happened, and refuses to share. Tonya tells Natalie to conduct a rape kit, including a pelvic exam. Neela refuses the exam and Neela and Tonya argue. Tonya insists that they conduct the exam, despite Neela's refusal, even though Natalie tells Tonya that this will require physically restraining Neela if she doesn't co-operate, which could traumatize her further. Tonya tells Natalie to sedate Neela and conduct the exam while she is unconscious, which Natalie refuses to do. Tonya insists that she will not let the people who hurt her daughter get away. Natalie tells Tonya that she cannot force Neela undergo a rape kit, and Tonya asks them to discharge her so she can find someone who will. Tonya confesses to Natalie that she is deeply upset because she survived being raped at Neela's age, and didn't tell anyone what happened. She wants to protect Neela. Natalie tells that Tonya that she should confide this information to Neela, and it might convince Neela to take their help. Tonya cries. Tonya takes Natalie's advice, and Neela in turn confides in her mother that she was raped at a party. Neela consents to the pelvic exam, and Tonya reassures her, telling her that what happened was not her fault, when Neela expresses feelings of guilt.
Meanwhile, FBI Agent Ingrid Lee and Jay Halstead prep Will for the meeting with Ray Burke, giving him a microphone to plant in Ray's house. Will is hesitant, expressing concerns about working with Ingrid instead of Jay, about hiding the microphone, and about the fact that Ray knows that Will's brother is a cop. When Ingrid expresses doubts about Will's commitment, Jay assures her he is ready. Later, at Ray Burke's house, Will attaches leads to Ray's chest to examine him, while Ray's sons Tommy Burke and Tim Burke watch. Ray tells Will that Will seems a little tense, which Will blames on too much coffee. Ray tells his sons that Will has wedding jitters. Will asks Tommy to activate the EKG machine, and attempts to place the mic when Ray grabs his hand and asks Will for a final headcount for the wedding venue. Will is startled, but manages to place the mic below Ray's chair while crouching next to him, and leaves, telling them he will show the EKG result to a cardiologist. Will returns to the hospital, but is intercepted in the parking lot by Ingrid, who tells Will he was supposed to check in as soon as he was done. Will tells her the entire process didn't go as predicted, but Ingrid tells him that the microphone was successfully planted and is giving them good audio. Will tells her he is uncomfortable with betraying his patient, Ray, as a doctor, and lying to his fiancee, Natalie. She tells Will to tell Natalie part of the truth, that he was paying a house call to Ray, without disclosing the details of the investigation. Ingrid reminds Will that Ray is a terrible person, and that he cannot tell Natalie about the investigation. Natalie sees Will replacing the EKG machine, and he asks her what she missed. She indicates that she doubts Will's earlier excuse of running errands.
Later, Will is intercepted by Tommy Burke and Tim Burke while he is getting some food from a vending machine. Will tells them that Ray is waiting for him outside, and Will tells them it is not a good time. They insist, and Will goes with them. Natalie sees them leave. They take Will to the far end of the parking lot and force him into car with Ray. Ray thanks Will for seeing him again and tells him to get in the car. In the car, Ray tells Will that he wasn't happy with the exam earlier, and that he didn't want to say anything in front of his sons earlier. He tells Will he is having dizzy spells, and notices that Will seems distracted, while Will sighs in relief that he hasn't been caught planting the mic. Will tells him not to get anxious, and to wait until the EKG results come. Ray asks if Will thinks it could be serious, and Will promises Ray he will find out what is wrong.
Connor tells Ava that the board refused his proposal for a dedicated bypass machine in the hybrid O.R., and also refused to disclose the identity of the anonymous donor. He asks Ava for help in finding out who the donor was, as he noticed that at mixers that she has the donors wrapped around her finger. He asks Ava to "do this one thing" for him, which infuriates her. She tells Connor he doesn't need a bypass machine, and any such cases should be brought to her in the cardiac wing. Ava tells Connor he has everything he needs in the hybrid O.R., and to stop grasping for more and appreciate it. Connor is confused by the intensity of her anger.
Meanwhile, Dr. Charles sits with Keith and urges him to see his son, which Keith refuses. Keith tells Dr. Charles he feels hopeless and that he doesn't want his son to feel the same way that he did when he got his diagnosis. Max bursts into the room, while nurses and Ava try to restrain him. Max begs Keith to seek treatment, telling him not to refuse help, and that he wants Keith to meet his fiance. Keith dies, while Max urges the doctors to save him, even as they tell him Keith has refused lifesaving help. Ava calls the time of death, apologizing to Max. Later, Max asks them what Keith was trying to hide, as Keith had asked for his medical records to be sealed. Max tells them he won't be that way with his kids. Ava and Dr. Charles cannot tell Max about Keith's diagnosis, but Dr. Charles tells Max that he can ask the medical examiner for an autopsy. This will enable Max to find out the truth about the possibility that he has Huntington's, especially since he plans on having children, without having any of them violate confidentiality rules. Dr. Charles tells Ava that maybe there are some secrets we don't get to keep.
Ava goes to speak to Connor in the new hybrid O.R. Connor tells her that he was looking for her as well, and that he wanted to apologize to her, as she was right about how many people worked hard to make the O.R. happen, and that he shouldn't be asking for more. He tells Ava that he's glad it happened, because he was looking for an excuse not to go Mayo, as he didn't want to leave her. Ava seems about to tell him something, but instead informs him that they lost Keith. She returns to complete some work, and Connor does as well.
Later that night, Natalie sees Will examining an E.K.G. reading. Will tells it belongs to a patient who clearly has significant blockages in his heart. Natalie tells him he should follow up with an echo, and asks who the patient is. Will lies again, and tells her he doesn't know, and that one of the med students asked him to look at it. Natalie reminds him that they talked about communication at the pre-wedding counselling, and asks about the mysterious errands, the two men she saw him leaving with, and that she doesn't want secrets between them. Will confesses that the EKG belongs to Ray, that he had gone earlier to pay him a house call, and not run errands, and that the two men she saw were Ray's sons, who were following up with him. Natalie asks why he wouldn't just tell her that, and Will tells her he feels guilty about what he is doing, by using him to get the wedding venue. He apologizes to Natalie and she forgives him.
