Neil Dietrich is an insane patient who was brought to Gaffney Chicago Medical Center.

Neil suffered a drug overdose in the past and came to believe he was in a game and nothing was real. He was soon sent to the Gaffney Chicago Medical Center, due to a ruptured appendix, but refused treatment, believing that he was in a simulation. Drs. Choi and Archer reached out to Dr. Daniel Charles to help on a psych evaluation on Dietrich. When he was passed out, Archer went on with the operation, consent or no consent. He later spotted both Choi and Archer in the parking lot and drew a gun on them, accusing them of trapping him in what he believed to be a game. Ethan attempted to disarm him, only to be shot. Before he could do more harm, Sharon Goodwin promptly struck him with her car, knocking him out. Goodwin explained she had witnessed the confrontation from afar and had to intervene. Both Neil and Choi were treated for the injuries, surviving their attacks. Neil was later admitted to a psychiatric ward, still holding onto his delusional belief.
