All the Lonely People is the tenth episode of Season 4 of Chicago Med.

Summary[]

April, Doris and Monique find themselves in a dangerous situation. Natalie receives an update on Will. Dr. Rhodes gears up for a gala honoring his hybrid O.R. and tries to convince Dr. Bekker to join him, to unexpected consequences.

The episode opens at Dr. Connor Rhodes' house, as he and his girlfriend, Dr. Ava Bekker, wake up together. Connor tries to persuade Ava to attend a large gala with him that night. The gala is to honor the new hybrid O.R. established at Chicago Med, which was designed and is run by Connor. Connor points out that unless Ava had persuaded Connor's wealthy father, Cornelius Rhodes, to fund the O.R., it could not have been established, but Ava refuses to go, telling Connor to go ahead without her. Meanwhile, nurses Doris, Monique Lawson, and April Sexton are arriving at the hospital, discussing the fact that April's ex, Dr. Ethan Choi is now dating his former girlfriend, Dr. Vicki Glass. Just then, a man driving by in a car shoots at them several times, and crashes his car, falling out of the driver's seat. April shields Doris and Monique, getting them down on the ground, and then kicks the gun away from the shooter's hand. She calls 911 to report the shooting to the police.

Inside the E.R., charge nurse Maggie Lockwood and Dr. Ethan Choi rush to check on April, who assures them she is fine. The shooter, Dennis Mitchell, is brought in for treatment as well, and is screaming death wishes at the staff and at female staff, particularly. Hospital administrator Sharon Goodwin tells Doris, April, and Monique that they can take the day off, given the traumatic incident that just occurred, but April and Monique elect to stay and help. Dr. Natalie Manning treats April's injuries, and clears her to return to work at April's request. Dr. Choi attends to Dennis Mitchell, the shooter, along with Dr. Daniel Charles, the Head of Psychiatry. They discover that Dennis has had a lot of plastic surgery, and Ethan orders a wide range of tests. As they stabilize Dennis, he continues to yell abuses, telling them that "they all deserved it," and "they'll learn." He is particularly aggressive towards women, calling a female police officer, Kim Burgess a "bitch" and "feminoid". As they wait for test results, Dr. Daniel Charles sees medical student Elsa Curry, who is struggling with her Ph.D. project, which has hit a wall. Dr. Charles suggests to Elsa that she should take a break and come help him assess Dennis Mitchell instead. Elsa agrees. Dennis is aggressive towards Elsa, asking Daniel to make her leave and calling her a "foid". While Dr. Charles orders him to treat Elsa with respect, and asks Dennis for his explanation. Dennis tells them that the women he attacked 'deserved it' and that women won't be with him, even though he claims that he deserves it more than what he calls "alpha males". Later, Dr. Charles tells Elsa that Dennis is using certain terms common in misogynistic hate and incel groups online ('Stacy', 'foid' and so on). He asks a reluctant Elsa to do some research into these terms.

As Dr. Choi works, his girlfriend Dr. Vicki Glass comes to the hospital, having heard about the shooting. On learning April was involved, but is unhurt, she asks Ethan if he is still emotionally involved with April. Ethan assures her that their relationship ended long ago, and makes plans with Vicky for dinner. Seeing April back at work, Dr. Choi is concerned, telling April to go home and rest, but she brushes it off. Meanwhile, Maggie Lockwood, Monique Lawson and Dr. Charles check patient records, and realise that Dennis Mitchell was previously a patient at the hospital, having contracted an infection after his plastic surgery. Monique recalls that she was his nurse. Dennis' file shows that he made inappropriate advances towards Monique, following which she asked to be replaced by a male nurse. As student doctor Terry McNeal and Dr. Choi prepare to discharge Dennis into police custody, they notice that his arm has lost blood supply, meaning they need to keep him in the hospital and evaluate the injury. Meanwhile, student doctor Elsa Curry has completed the research on Dennis Mitchell's online presence and sits with Dr. Charles to explain it to him. She tells Dr. Charles that Dennis describes himself as an 'incel' and that he has been radicalized into misogynistic hate groups. Dennis has posted a video online talking about his hatred of women, and his plans to slaughter them online, as he is a 28-year-old virgin and resents women for not sleeping with him. Dr. Charles tells Elsa that loneliness is an epidemic.

In a conversation with Dennis, he tells Dr. Charles and Elsa Curry that he doesn't feel any regrets, and, if given a chance, he would do it again. This infuriates Elsa, who berates Dennis. She is taken outside by Dr. Charles, who tells her that she cannot yell at patients who are being evaluated. Elsa accuses Dr. Charles of trying to give Dennis a way out of paying for his crimes, and tells Dr. Charles that she doesn't understand what he expects of her, before leaving. As they go, Ethan Choi and Terry McNeal inform Dennis that his charts show that one of his pectoral implants caused an aneurysm which could rupture, causing Dennis to bleed to death. They send him for surgery immediately. April lets Ethan know that the surgery went well, sarcastically congratulating him for saving her attacker's life. Ethan tells April that Dennis hurt someone he cares about - April - and that Ethan's own first impulse was to take revenge, but he quashed it, because it would make him no different from Dennis himself.

Dr. Natalie Manning is called to Sharon Goodwin's office, where she meets F.B.I. Agent Ingrid Lee. Ingrid Lee had taken Natalie's fiancee, Dr. Will Halstead, into protective custody on their wedding day, preventing the marriage. Will, unbeknownst to Natalie, had been acting as a confidential informant in the F.B.I. and Chicago P.D.'s investigation of Ray Burke, who was Will's patient. Natalie expresses her frustration with the situation to Agent Lee, as she has not been able to speak with Will since the incident. Agent Lee, in turn, tells Natalie that Will's information enabled them to arrest Ray, a criminal, and that now that Ray is imprisoned, they are releasing Will from protective custody. Agent Lee warns Natalie that people who have been in protective custody may take time to readjust to their normal lives, as they have faced isolation and trauma. Back at the E.R., a pregnant woman, Ivy Davis, is brought in with contractions. Her husband, Phillip Davis, tells Dr. Manning and nurse Monique that Ivy has a pre-existing aneurysm and that the exertion of a natural birth could kill her, which is why they have a c-section scheduled. Natalie attempts to slow down Ivy's contractions with medication, but finds that it may be too late, as the baby is ready to be born. She pages up to obstetrics for doctor, but as they are unable to send anyone at once, performs the c-section herself, successfully delivering the baby. However, Ivy dies during the birth, due to her pre-existing aneurysm. The baby girl, Davis is moved up to neonatal intensive care.

In neonatal intensive care, the baby girl, Sophie Davis is found to have been born with a heart defect, missing her tricuspid valve and needs urgent surgery. Phillip Davis consents, and begs Natalie to save Sophie. Maggie then sends Natalie into another trauma bay, where she sees that her fiancé, Dr. Will Halstead, has returned from protective custody. Natalie tells Will that she has been furious with him for all the lies and deception, and Will apologizes. Their reunion is awkward, and Will is visibly traumatized, but they eventually embrace and kiss. Will tells Natalie he thought about being back with her everyday, and Natalie tells Will she missed him. Later, sitting in the break room, Will is startled when Ethan enters, spilling his coffee as a result. Will tells Ethan that he's still a little jumpy, as Ethan welcomes him back. He tells Ethan that he was kept in protective custody in Phoenix, but wasn't allowed outside much. Natalie tells Will she just needs to finish some paperwork, and then they can leave. As they sit, Natalie tells Will about Phillip Davis, and that she knows how he feels, being a newly single parent with a newborn child. Will and Natalie leave, and Natalie stops on her way out to give Phillip Davis some information about funeral homes for his wife. She tells him to go home and get some sleep, but he refuses, wanting to stay with his daughter. Natalie tells him she was pregnant when her husband died, and it made her whole world fall apart, describing her grief and loneliness. She promises him it will get better, but Phillip collapses, sobbing, and Natalie comforts him as Will watches.

Dr. Connor Rhodes is assigned by the Head of Emergency Services, Dr. James Lanik, to treat a child, Davey Bennet. Davey accidentally put a nail in his chest while playing with a nail gun, and Davey's father, Mr. Bennet, apologizes, telling Connor he thought the nail gun was out of reach. Connor decides to place Davey under anaesthesia to remove the nail, as he is still a child and may not be able to hold still as an adult would. Davey is very nervous about the operation, and Mr. Bennet insists on accompanying his child into the O.R. Although this isn't always allowed, Connor, feeling sorry for him, lets him be there. During the surgery, complications arise and Mr. Bennet panics and starts screaming, accidentally pushing a tube into Davey's chest, which lacerates a heart vessel. He is forcibly removed from the E.R.. Connor removes the nail successfully, with pages Dr. Ava Bekker, who fixes the tear. Later, Mr. Bennet apologizes to Connor, telling he was right to tell Mr. Bennet to stay outside and wait. After the surgery, Connor again asks Ava to come with him to the gala that night, but she tells him that she doesn't want to, because Connor's father will be there along with the rest of the board. Ava says it is uncomfortable being around both of them at the same time. Connor tells Ava that she is his girlfriend, and he hopes they have a future together, so she can't avoid his father forever.

Later, Elsa Curry questions Dr. Charles about the medication he prescribed for Dennis Mitchell, their incel shooter patient. Dr. Charles asks her to stop by his office later in the day. In Dr. Charles' office, he tells Elsa that men tend to blame others for their depression, while women turn the blame inwards. He tells her that she had a very strong reaction to Dennis Mitchell. He tells her that he is concerned, as Elsa has described the symptoms of stress and depression to him, including a lack of sleep, irritability, and so on. He offers Elsa help, and she tells Dr. Charles he has a problem, not her, pointing out his several divorces, the fact that his former student, Sarah Reese, left, and tells him that he is projecting his own issues on everyone else. She leaves, slamming the door.

Ethan leaves the hospital, and he and April say good night to each. April tells Monique to go home, as her shift is over, but Monique apologizes to April, blaming herself for the shooter's actions, and crying. Maggie and April take Monique to the break room, throwing out all the doctors sitting there and comfort her. Maggie and April tell Monique that her rejection of Dennis Mitchell does not mean she is to blame for his actions. Finding out that she has no relatives or friends available, Maggie tells her that she does have friends, gesturing to herself and April.

Later, at home, Natalie tells Will that they need to start planning a wedding again, and is taken aback when Will starts unpacking his belongings, revealing a gun. Natalie makes it clear that she doesn't want a gun in her house, even as Will insists that he is duly licensed, and that they need it after everything that happened. Will tells her he used to be opposed to guns until he had one pointed at his head, but when Natalie refuses to have a gun in her house, he packs up his stuff and leaves.

At the gala, hospital C.O.O. Gwen Garrett thanks the hospital's donors in a speech, as Connor and Ava watch. Gwen takes Connor aside, and tells him that when the press try to raise questions about the shooting incident earlier that day, he should deflect and talk about the new hybrid O.R. Meanwhile, Connor's father, Cornelius Rhodes, sees Ava standing alone and goes over to her, complimenting her dress and perfume. Cornelius tells Ava that the smell of her perfume brings back nice memories, and asks her to repeat them, telling her that he wrote her a very large cheque. He asks Ava if Connor knows about them. Panicking, Ava asks Connor to leave, and when he presses her on what happened, she tells Connor that Cornelius is spreading rumors that Ava slept with him. Connor punches Cornelius at the gala, in front of everyone, shocking everyone. Cornelius asks Connor what Ava told him, making it clear he doubts her. Connor and Ava leave, with Connor telling Ava he should never have made her come to the gala with him.

At the end of the episode, Dr. Charles is seen sitting alone outside the hospital, watching people go by.
